Exam Entry (ABRSM)

If your child has been invited to take a music exam, please complete this form by the deadline given in the email to give permission for me to enter you/your child to the ABRSM. 

Current exam period:
Exams taking place in June/July, 2020
Deadline for parental consent and payment: 17th April, 2020

    Exam Entry Form (ABRSM)
    By ticking the consent box below, I give permission for Philip Anderson to enter me/my child (named above) for their ABRSM exam. I consent to the following details being passed over to the ABSRM for this purpose:
         - Student's Name
         - Student's Date of Birth
         - Any medical condition or special requirements that will affect the exam (eg dyslexia, autism, need for large print, etc).
    I understand these details will be taken from the data Philip J. Anderson Brass Tuition already holds on me/my child and that if this information has changed I will also complete a new Personal Details form and return this by the deadline.

    I understand this form AND full payment, plus an updated Personal Details form where applicable, MUST be received by Philip by the deadline of 17th Jan, 2020 in order to be entered into the March/April, 2020 exam period.
